This is another easy one that might not be readily apparent...  In the circular room with a button in the middle and another button behind the low spikes, pull the lever and smash through the broken wall with the dead body.  Smash through the opposite wall as well (behind the low spikes).  You will find a dead body by the first wall (after defeating enemies). 

Drop the dead body in the little river then run across the room and pick it up as it floats to you.  Place your dead friend on the button to open half of the exit door, then stand on the button in the middle of the room to open the other half of the door.  Run through and save your game.
